The purpose of the Community Support Program (CSP) is to assist individuals who have long term mental health problems to live as successfully and independently as possible in their own communities.

CSP will work together with clients one to one in their homes and communities to develop their self reliance thought the teaching of independent living and coping skills.  It is the aim of this program that through this teaching, clients will become more skillful in dealing with life's goals, problems and demands.  CSP Case Workers will provide services based on goals developed buy the client, CSP Case Worker, and Individual Community Support Plan.

There is no charge to the individual for the CSP program as it is funded through a state grant (Rule 14).

Services provided include:

  • Client Outreach     
  • Crisis Assistance
  • Housing and Benefits Assistance
  • Employability Services
  • Medication Management
  • Psychosocial Rehabilitation
  • Assistance in Independent Living Skills

After a referral has been made to the CSP Program, a CSP Case Worker from Western Mental Health Center will make contact with the individual to introduce our services.  Upon acceptance of the services, the CSP Case Worker and the client will work together to assess the client's needs and develop a program plan. 

Based on the client's individual goals and needs, the CSP Case Worker will focus on helping the client develop the coping skills necessary to live as successfully and independently as possible in the community in an attempt to minimize the need for inpatient treatment.
